Typos in initValue/shiftIdx Tables

Table 123 has a typo in the ctxIdx of sig_coeff_flag

• ctxIdx 128-143 is repeated on two rows
• ctxIdx 144-159 is omitted

It appears that the second 128-143 row is intended to be the 144-159 row.

Table 125 lists ctxIdx 112-127 at the bottom of p.413, then has a separate header listing ctxIdx 0-15 at the top of p.414. The second 0-15 ctxIdx list appears in error.

The second instance of 128-143 should indeed read 144-159 in table 123.
As for table 125, this seems to be an issue with the published version of H.266 where table headers are repeated when a table spans multiple pages (JVET-T2001v2 looks ok).

Thanks! Actually I found that the same issue also exists in JVET-T2001-v2. Anyway, this will be fixed in the VVC attachment of JVET-Y1004, the ITU-T VVC v2 text, and the ISO/IEC VVC v2 FDIS text.
